About Carol Claire

Carol “Claire” Crisostomo is a board-certified Family Nurse Practitioner passionate about caring for the whole person—mind, body, and spirit. Her nursing journey began as a CNA and grew through roles as an LPN and RN, giving her a strong foundation in patient care and empathy at every stage of health.

Claire graduated summa cum laude with her Associate Degree in Nursing from Northern Virginia Community College, earned her Bachelor of Science in Nursing from Grand Canyon University, and completed her Master of Science in Nursing – Family Nurse Practitioner at Saint Joseph’s College of Maine. Her academic excellence reflects her commitment to lifelong learning and advancing her ability to provide high-quality care.

As a provider, Claire focuses on partnering with patients to help them achieve their health goals—whether that means improving wellness, managing chronic conditions, or maintaining a healthy lifestyle. She is dedicated to delivering personalized, holistic care that supports each individual’s unique needs. She is also a proud member of both the American Association of Nurse Practitioners (AANP) and the American Nurses Association (ANA).

Outside of her practice, Claire is a proud military-veteran wife and devoted mother of three. She enjoys traveling, discovering new recipes, watching movies, listening to music, resting, and spending time with her family. Her values of service, resilience, and compassion guide her both at home and in her work with patients.
