Vitamin B12 Deficiency: Do You Need B12 Injections or Oral Supplements?

Vitamin B12 deficiency can cause fatigue, numbness, tingling, memory issues, poor balance, sleep problems, and anemia. Treatment depends on your symptoms, your B12 level, MMA and homocysteine testing when needed, and whether your body can absorb B12 through the stomach or needs B12 injections.
B12 Levels: Why Symptoms Matter More Than the Number Alone
In clinical practice, we do not treat only a number. We treat the patient, the symptoms, the history, and the overall presentation.
Usually, when B12 levels are very low, especially in the 0 to 200 range, patients are more likely to show hematological effects. These are blood-related effects, such as large immature red blood cells, abnormal red blood cell maturation, and anemia.
However, many patients can have neuropsychological symptoms at higher levels, commonly in the 0 to 300 range. These symptoms can include anxiety, sleep issues, fatigue, numbness and tingling in the arms, legs, hands, fingertips, or feet, poor sense of touch, poor balance, memory issues, and brain fog.
When symptoms are concerning but the serum B12 is not clearly low, or the results do not match the clinical picture, the way around the serum B12 level is to check methylmalonic acid, called MMA, and homocysteine. These tests can help confirm or exclude vitamin B12 deficiency when the B12 number is borderline low and the patient agrees to additional testing.
So if your B12 is borderline low, especially around 300 or less, and you have numbness, tingling, fatigue, poor balance, memory issues, or nerve-type symptoms, we may order MMA and homocysteine as well.
A patient with a B12 level of 300 and no symptoms may be very different from a patient with a B12 level of 300 who has numbness, tingling, fatigue, poor balance, and memory issues.
The lab number matters, but the patient matters more.
Medication-Related B12 Deficiency
Some medications can contribute to B12 deficiency, especially when used long term.
Common examples include metformin, used in diabetes care, and proton pump inhibitors, also known as PPIs. PPIs are acid-reducing medications commonly used for acid reflux or GERD.
If you are taking metformin or a proton pump inhibitor and your B12 is low, oral supplementation can be fine for many patients, especially if the deficiency is mild and there are no major neurologic symptoms.
If the medication is still needed, we usually continue monitoring and treating the B12 deficiency instead of ignoring the symptoms.
Intrinsic Factor Deficiency
Intrinsic factor is a protein that is necessary for proper B12 absorption.
A simple way to understand intrinsic factor is to think of it like a hand or glove that grabs B12 and helps bring it into the body. If this hand is missing or not working properly, B12 cannot be absorbed normally.
Patients with intrinsic factor deficiency can develop pernicious anemia. These patients are more likely to have long-term B12 deficiency and may develop more significant symptoms over time.
If the deficiency comes from intrinsic factor deficiency or pernicious anemia, then oral B12 is usually not the correct main treatment, because the body cannot absorb B12 properly. In these patients, B12 injections are recommended, especially if they have anemia, neurologic symptoms, or very low B12 levels.
Oral B12 Versus B12 Injections
B12 can be supplemented in different ways. Some patients do well with oral B12. Some do better with injections.
Oral B12 may be appropriate when the deficiency is mild, the patient does not have significant neurologic symptoms, and the body can absorb B12 adequately. This can include many patients whose B12 deficiency is related to diet, metformin, or PPI use, depending on the level and symptoms.
B12 injections may be preferred when the deficiency is more severe, symptoms are significant, absorption is poor, intrinsic factor deficiency is present, pernicious anemia is present, or neurologic symptoms such as numbness, tingling, burning, balance issues, poor sense of touch, or memory problems are present.
The decision is not the same for every patient. It should be based on symptoms, lab results, medical history, MMA and homocysteine when needed, response to prior treatment, and the suspected reason for the deficiency.
When Should B12 Levels Be Rechecked?
Timing matters.
If you check B12 levels right after an injection, the number may look very high, but that does not always tell us how long the level is lasting in the body.
A better approach is not to draw blood right after the injection day. In many patients, we wait about 28 days after the injection and then check the level. This gives us a better idea of whether the B12 level is still holding in a good range before the next dose.
We often want the B12 level to stay above 300 in symptomatic patients. If the level drops too low before the next injection, or if symptoms are not improving, the dose or frequency may need to be adjusted.
If symptoms are concerning and the serum B12 level does not clearly explain the clinical picture, we may also check MMA and homocysteine to see if there is functional B12 deficiency.
Again, the goal is not just to make the lab number look normal for one day. The goal is to make sure the patient feels better and the level stays adequate over time.
